We complete benefits checks via our 3rd party biller however please check with your insurance provider prior to your appointment(s) to be informed of any deductibles and/or co-pays. If you do not see your insurance provider listed, check with your insurance provider if you have out-of-network benefits. If you choose to pay out-of-pocket, please call our office directly to discuss rates. We may offer a sliding scale based on your specific financial situation.
