Please note that we charge a $100.00 refundable deposit (via credit, debit, or HSA card) in order to schedule your initial intake appointment. This is to ensure appointment security. This deposit is fully refunded at the completion of your final appointment or fully refunded if you choose to cancel the remainder of your evaluation with more than 48 hours notice. It is not refundable if you cancel your appointment with less than 48 hours notice.

We complete benefits checks via our 3rd party biller however please check with your insurance provider prior to your appointment(s) to be informed of any deductibles and/or co-pays. If you do not see your insurance provider listed, check with your insurance provider if you have out-of-network benefits.

If you choose to pay out-of-pocket, please call our office directly to discuss rates. We may offer a sliding scale based on your specific financial situation.
